A student who graduated from Illinois State may order a new undergraduate or graduate diploma using the Diploma Replacement Form found at http://www.registrar.ilstu.edu/graduation/diploma.shtml.
If you want notarized copies of your diploma signed by the University Registrar and Notary Public, please indicate so on the Comments: line on the form.
If you want the notarized copies of your diploma to also include an Apostille, you must include the application (link below) or a letter addressed to the Illinois Secretary of State requesting an Apostille on each certified diploma and stating to which country the Apostille will be sent. Also include a check or money order made payable to the Illinois Secretary of State including $2.00 for each document needing an Apostille. We provide two notarized copies of the diploma, so $4.00 would be required by the Secretary of State. Include a pre-addressed, postage-paid envelope with your request to the Secretary of State so that they may mail the notarized diplomas with attached Apostilles to you or a third party.
