Steps to Receive VA Education Benefits

  1. Apply for admission to the University.
  2. Register for classes.
  3. Contact Veterans Services for appropriate forms.
  4. Complete required paperwork for the GI Bill or other education benefits.

Please download and complete any of the forms below that apply to your situation:

First Time Recipients

The following forms are required by Veterans Services:

Members of the Montgomery GI Bill/Prior Active Duty Programs

  1. Application for Educational Assistance (VA Form 22-1990) or Disabled Veterans Application for Vocational Rehabilitation (VA Form 28-1900) if appropriate.
  2. Photocopy of DD-214 Discharge Document, Member 4. This form is provided to you by your military branch at completion of enlistment.
  3. Illinois State University Request for Veterans Benefits

Members of the Montgomery GI Bill Selected Reserve/National Guard

  1. Application for Educational Assistance (VA Form 22-1990)
  2. Copy of DD Form 2384 Notice of Basic Eligibility (NOBE). This form is provided to you by your Unit.
  3. Illinois State University Request for Veterans Benefits

Dependents or Spouse of 100 percent Disabled or Deceased Veterans (Service Connected)

  1. Application for Educational Assistance (VA Form 22-5490)
  2. Illinois State University Request for Veterans Benefits

Transfer, Readmitted, and Continuing Students

  1. Illinois State University Request for Veterans Benefits
  2. Other appropriate forms will be provided to you by Veterans Services
